Checklist for reorganizing teams into stream-aligned units.
- Map end-to-end customer journeys, value streams, and supporting systems; highlight current bottlenecks and ownership gaps.
- Draft team charters with mission, scope boundaries, KPIs, and collaboration expectations; secure leadership sign-off.
- Align product backlog, roadmap, and on-call coverage to each stream team; re-route intake processes accordingly.
- Define interfaces with platform, enabling, and subsystem teams (APIs, SLAs, support channels, escalation paths).
- Communicate changes to affected staff; provide transition FAQs, training plans, and pairing opportunities.
- Update service catalog entries, access permissions, and operational documentation to reflect new ownership.
- Run transition retrospectives after two and six sprints; adjust scope, staffing, or support based on feedback.
- Track outcomes (lead time, flow efficiency, satisfaction) and share progress with executives and partner teams.
